I've started a solo cardiology practice in a poor community with a lot of Medicare/Medicaid and uninsured patients. While my practice has achieved healthy growth, a few family medicine doctors and internists in town have become accustomed to practicing without a cardiologist. They have not referred as many patients as I expected. Moreover, they are very resistant to having a one-to-one conversation.
